Download the HOOF form—save time and lives! Just before the 2020 firestorm, horse evacuation emergencies was at the top of our list! BCHA was working with Brandy Perkins, Animal Control Officer with the Boulder County Sheriff’s Office to revise the Horse/Livestock Operation Find (HOOF) Form. The Boulder County Sheriff’s Office is the central dispatch for emergency evacuation and lost and found large animals.

When you fill out and submit the H.O.O.F. form to Brandy Perkins, your information will be maintained in a central database for horses and other livestock. This will allow the Boulder County Sheriff’s Office to quickly contact you during an emergency.
